穏やかで青く晴れた空（the blue）。
良い天気だな〜♪と思っていたら、いきなり雷が「ピシャーン！」
そんなところからの比喩^*1で、out of the blue は「なんの前触れもなく突然に」 suddenly and unexpectedly ということを表すイディオム表現。
日本語の慣用句にある「青天の霹靂（へきれき：雷のこと）」と同じイメージですね。
out of a clear [blue] sky でも同じ。

In the middle of class, Greg started to sing out of the blue.
「授業中、グレッグは唐突に歌を歌いだした。」
